- comments: observed a food handling employee put ingredients on a taco (onion and cilantro) with their bare hands. Management instructed that no bare-hand contact is allowed with ready-to-eat foods. Priority 7-38-010. Citation issued.
14. Required records available: shellstock tags, parasite destruction
- comments: observed no shellfish tag for oysters. A shellstock certification tag shall be maintained with the original container until emptied and then retained for 90 days from the date of the last one sold. Tags must be with the product at all times, and shellfish tags must be dated with the date when the last shellfish was used or consumed from the container. Management discarded the said oysters. Total value: $30. 00 total weight 10 lbs. Priority foundation violation 7-38-005 citation issued
21. Proper hot holding temperatures
- comments: observed al pastor held at an inadequate temperature of 75 f. Meat was not cooking and sitting stationary on a rotator instructed the manager to ensure that al pastor is either cooking or stored at 135 f or above. The manager discarded 10 pounds of el pastor $60. Priority violation 7-38-005 citation combined with violation #22.
22. Proper cold holding temperatures
- comments: observed sliced tomatoes and milk stored at an inadequate temperature of 47 f. Instructed manager to ensure that all cold food is stored at 41 f and below. The manager discarded 1/2 pound of tomatoes and 3/4 gallon of milk. Priority violation 7-38-005 citation issued.
36. Thermometers provided & accurate
- comments: observed no food thermometer on site for measuring cold foods. The food thermometer on site starts at 100 f and up. Instructed manager to provide metal stem thermometer for measuring hot and cold foods. Priority foundation violation 7-38-005 citation issued.
38. Insects, rodents, & animals not present
- comments: observed 25 small flies throughout the facility, including in the basement, in the kitchen, and toilet rooms; observed 1 live roach in the kitchen and 1 live roach on cooler in drink area. Instructed manager to service all areas affected by pests priority foundation violation 7-38-020(a). Citation issued.
38. Insects, rodents, & animals not present
- comments: observed dead rodent in basement near hand washing sink. Instructed manager to remove dead rodent and sanitize affected area.

- comments: observed food stored on the floor in the basement. Instructed manager to keep food items at least 6 inches above ground.
43. In-use utensils: properly stored
- comments: observed knife stored in crack between wall and counter instructed manager to remove knife in store on a non-absorbent easily cleanable surface.

- comments: observed mop sink with clog. Instructed manager to unclog mop sink.

- comments: observed food handler with no food handler training. Instructed manager to ensure all food handlers have food handler training.

- comments: observed previous core violations not corrected within specified timeframe. Report #2563240, dated september 6, 2022, instructed facility to: #37 label food storage containers with common name, #47 repaint rusty metal shelves and racks in basement walk in cooler, #51 provide backflow prevention device to hose in basement, and #58 provide food allergen training for all certified food managers. Observed bulk storage containers in basement not labeled with common name; observed rusted metal shelves and racks in basement walk-in cooler; observed hose in basement hooked up with no backflow prevention device; observed certified food manager with no food allergen training. Instructed manager to address all previous core violations. Priority foundation violation 7-42-090 citation issued.
