Report date: Mar 18, 20252. City of chicago food service sanitation certificate
- comments: observed facility without a certified city of chicago food service sanitation manager onsite while tcs foods such as beef, tuna, etc are prepped and served. Person in charge presented a certificate as his was was presented to me back in january by a female worker. An id could not be presented to match the certificate posted which was not her. Informed person in charge city of chicago original food service certificate must be posted along with the certified manager on site when time and temperature control for safety(tcs) foods are stored. Priority foundation violation #7-38-012. Citation issued.
52. Sewage & waste water properly disposed
- comments: observed waste water backing up/spilling onto floor from floor drain when draining 3 compartment sink at time of inspection. Instructed manager to get pipes cleaned to prevent back up onto floor of facility. Priority. 7-38-030(c). Citation issued. Manager was able to fix issue while i did my report
