- comments: 2-101. 11 : observed no valid certified food managers certificate posted to view. Must provide and maintain. (priority foundation 7-38-012)(citation issued)
10. Adequate handwashing sinks properly supplied and accessible
- comments: 5-204. 11 : observed no hand washing sink in rear dish washing area. Must install a hand sink with hot and cold running water,soap & towels and maintain. (priority foundation 7-38-030(c)(citation issued)

- comments: 7-206. 1: observed evidence of 4- packaged and bloxs of toxic rodenticide (poison)scattered throughout premises under and around coolers on sales floor,service counter shelf. Must remove poison and provide inside a closed container. (priority 7-38-015)(citation issued)

- comments: 4-302. 12 : observed no probe thermometer for taking internal food temperatures of tcs foods must provide and maintain. (priority foundation 7-38-005)(citation issued)

- comments: 6-501. 111 : observed an infestation of appx. 150 or more mice droppings scattered throughout premises such as under shelving units at service counter,inside of doors and open spaces underneath unused deli display unit,under all food and drink display shelves,under & in between all reach in coolers,in toilet room adjacent to dish washing area- on floor behind door. Basement-on ledges leading to basement,on concrete landings,on floor under & on most shelving units,in between massive clutter stored throughout basement. Also. Appx. 5 or more live small flies in rear prep areas near hand sink. A dead mouse on floor underneath a crate in basement. Must remove droppings,dead mouse clean and sanitize areas. Must have a pest control operator to service premises on or after 9/20/23. (priority 7-38-020(a)(citation issued)

- comments: 6-202. 15 : observed several gaps along sides of side door in dish washing area,basement screen on door not tight fitting and door is wide open as well as 1st fl. Front door must make doors tight fitting to prevent insect and rodent entry.

- comments: 4-101. 19 : must eliminate crates used for food storage throughout premises including basement and provide adequate shelving units

Must provide a typed written sanitizing procedure on how to propely sanitize utensils. Must remove excessive loose caulking from inside of 3- compartment sink basin and use food grade caulking only.
