- comments: observed no hand cleanser available at handsink located in (in use) prep area as well as rear prep and dishwashing area during time of inspection. Instructed to provide and maintain at all times while open and operating. Priority foundation violation. #7-38-030 (c). Citation issued. Pic provided at this time.

- comments: observed no hand drying provision available at handsinks located in meat prep and rear prep/dishwashing area. Instructed to provide and maintain at all times while open and operating. - comments: observed packaged red and green salsa stored inside reach in coolers without labels. Instructed to provide and maintain.
38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity on site. Approximately 45 small and large flies flying throughout front and rear prep area, on walls, equipment in place and on stored kitchenware. Noted 1 large adult cockroach crawling on floor near cooling units in rear prep/dishwashing area. Recommended to contact pest control for additional service. Must eliminate pest activity on premise priority foundation violation. #7-38-020 (a). Citation issued.
40. Personal cleanliness
- comments: observed food employee preparing meat in prep area without hair restraint. Instructed to provide and maintain.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed sliding doors on display cooling unit containing meat in disrepair. Instructed to remove all duct tape mending equipment together, repair and maintain in good condition.

- comments: observed tables, unused equipment and interior surfaces of cooling units in need of cleaning. Instructed to clean and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed damaged ceiling tiles above produce cooler. Instructed to repair and provide smooth and easily cleanable ceiling attachements.
56. Adequate ventilation & lighting; designated areas used
- comments: observed burned out light bulbs in ceiling throughout meat prep. Instructed to replace and increase lighting in said area.
