- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)(citation issued)

- comments: observed no hand washing sign at hand sink in bar area.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed excessive build up of black slimy substance on interior panel of ice machine and dripping on to ice inside of machine. Must remove ice,clean and sanitize machine and maintain. (priority foundation 7-38-005)(citation issued)

- comments: observed evidence of appx. 50 or more mice droppings on lower shelf of steam table,grill table,prep tables,on floor under cooking equipment and prep tables. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 12/1/22. (priority foundation 7-38-020(a)(citation issued)

- comments: observed a gap along bottom of west exit door must make door tight fitting.

- comments: observed ice scoop stored on top of ice machine in basement. Must remove and store in a clean container.
