- comments: observed evidence of appx. 50 or more mice droppings on floor underneath 3- compartment sink and in dry food storage area on floor and shelving units. Also. Appx. 6-8 live large flies flying around in rear prep/dish washing area. Must remove droppings clean and sanitize areas. Must have a pest control operator to service premises on or after 7/12/24. - comments: must repair or replace worn door gaskets on rear 2- door reach in cooler, damaged fan guard inside of 1- door reach in cooler and missing handles on hot holding units lids.

- comments: must clean grease and food debris build up from used and unused coolers,fryer cabinets, prep tables, steam table,service counter shelving units,interior and exterior of all cooking equipment.

- comments: observed inadequate sewage waste water backing up from floor drain and pooling onto floor in rear dish washing/prep area and in toilet room when water is turned on inside of 3- compartment sink basin. Instructed to provide signage to all sinks and maintain. #47- 4-202. 15 : observed the can opener in the rear with food and rust. Instructed to clean and remove rust from the can opener. Maintain. #47-4-501. 12 : observed the cutting boards with dark stains and deep grooves. Instructed to sand cutting boards or replace them. Must maintain a smooth and easily cleanable surface. :#49- 4-602. 13 : observed the rear prep tables and storage racks with rust, food and dirt. Instructed to remove rust, clean and maintain. #51-5-205. 15 : observed the washbowl in the toilet room slowly drains. Instructed to fix so that it drains properly and maintain. #55-6-501. 11 : observed the floors in the rear around the 3-compartment/hand sink and prep tables pitted and not smooth and easily cleanable. Instructed to make floors smooth and easily cleanable. Maintain. -observed water stained and damaged ceiling tiles in the rear prep area. Instructed to replace damaged ceiling tiles and maintain. #55-6-501. 12 : observed the floors in the corners, under/around heavy equipment and all sinks with thick grease, food, dirt and debris build up. Instructed to detail clean and maintain floors. :#56- 6-304. 11 : observed no ventilation in the toilet room. Instructed to provide mechanical ventilation and maintain. #57- 2-102. 13 : observed employees with no proof that they met the food handler's requirements. Instructed to have employees that are not certified food managers meet and provide proof they met the food handler's requirements. Maintain current proof. #58- 2-102. 13 : observed the certified food manager (cfm) with no allergen training/certificate. Instructed to obtain and provide. Maintain proof accessible to inspectors.
