- comments: found a food employee eating lunch soup at rear prep area. Instructed management that employees must consume beverages/food in designated areas to prevent contamination of food, equipment, utensils, etc.

- comments: observed personal item stored inside dry storeroom. Instructed manager to remove. Employees store items in designated areas where the contamination of exposed food or equipment cannot result.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the following sinks not maintained with hot water: improper temperature of hot water observed at handsink in front prep at (86. 5f). Instructed person in charge hot water at all hand washing sinks must be at least 100f. Priority violation #7-38-030(c). Citation issued.

23. Proper date marking and disposition
- comments: observed ready-to-eat, time/temperature control for safety (tcs) foods (tuna, chicken liver) was date marked to indicate the product's name and date in which the food must be sold. However, it has exceeded the discard. Date exceeding the dated tag on said product of chicken liver 5/30 and tuna 5/31. Pic discarded said food products. Priority foundation violation 7-38-005.

- comments: observed residential roach insecticide stored next to syrup in front prep area, and under cold holding unit. Informed that residential use insecticides are not permitted in commerical spaces. Priority violation. 7-38-005. Citation issued.

- comments: observed dead roaches throughout front and rear prep areas, under hot and cold holding area equipment, under stoves and cooking equipment. Instructed to remove all dead roaches and clean and sanitize said areas and equipment.

Over 50 live roaches observed in the following areas: front dining room, counter space, in booths in dining room, counter hot holding area, on cooking equipment, on the counter in cold holding area, rear prep area, dish washing area, and women's toilet room. 7-38-020(a) priority violation citation issued

- comments: observed food containers stored on the floor in the walk-in cooler, prep and storage areas. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

- comments: observed wet wiping cloths on several prep surface areas. Instructed to properly store wiping cloths in a sanitizing solution between uses to prevent contamination of equipment & food. Must maintain

- comments: observed plates not inverted. Instructed to invert plates after sanitizing and air drying.

- comments: observed cutting boards in the rear prep area with open seams and dark stains. Instructed to refinish or replace damaged or stained cutting boards and maintain at all times.

- comments: observed water-stained ceilings tile panels in the mens bathroom, and in rear area next to exit door instructed to repair, replace missing panels and/or replace the stained ceiling tiles.

Instructed to detail clean under all shelving units, interior of prep coolers, hot-holding units, prep tables and deep fryers to remove spills, food debris, dirt buildup and/or grease. - comments: observed chipped paint on the ceiliing of toaster area in front prep instructed to resurface and repaint chipped areas to ensure walls are sealed, smooth and easily cleanable.

- comments: observed the ventilation hood and filters above the cooking equipment in need of cleaning. Instructed to detail clean and maintain at all times.
