2. City of chicago food service sanitation certificate
- comments: no city of chicago certified food manager was on-site during the inspection while the temperature control for safety foods (sausage) was prepared and served. The certified food manager was not on site until approximately 9:25 am. Instructed to have a city of chicago certified food manager must be on-site at all times foods are prepared and served. Priority foundation violation 7-38-012, citation issued.

25. Consumer advisory provided for raw/undercooked food
- comments: observed incomplete consumer advisory on physical and online menus. Instructed to add an asterisk(*) next to all egg items on both menus because eggs are made to order. Priority foundation violation, no citation issued.

37. Food properly labeled; original container
- comments: observed bulk food storage containers in the prep area and in the rear prep area not labeled with common names. Instructed to label said containers and maintain.

40. Personal cleanliness
- comments: observed one food handler with out a hair restraint. Instructed management to ensure that all food handlers have on hair restraints while in the food prep area at all times.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed kitchenware (clean silverware, platic flat edge spatulas,bowls, pots) not inverted in the dishwashing area and thorughout food prep areas. Instructed to invert all said items to avoid contaminating said kitchenware.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed the interior of the deep frying unit under the ventilation hood with grease accumulation. Instructed to deep clean said unit and maintain.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed an accumaltion of debris(dunkin donuts coffee cups) near the outside receptacles. Instructed to clean and maintain said area.

55. Physical facilities installed, maintained & clean
- comments: observed flour and debris on the floor under the mixing station. Instructed to clean and maintain said area.

56. Adequate ventilation & lighting; designated areas used
- comments: observed missing filters under the ventilation hood in the prep area. Instructed to provide and maintain.

57. All food employees have food handler training
- comments: observed one expired food handler certificate on site. Instructed to have food handler retake course and provide and maintain records on site at all times.

60. Previous core violation corrected
- comments: previous core(s) violation(s) not corrected from inspection report#2500885 issued on 05/06/2021. #47 observed raw wood storage shelves for chemicals and dry storage. Must paint or seal shelves, so they are smooth and easily cleanable. #47 noted rusty and dirty shelves inside walk-in coolers in the kitchen prep area. Instructed to detail clean, repaint with non-toxic food grade paint, and/or replace. #55 noted floor of the beer walk-in cooler was made of raw wood. The floor needs to be painted or sealed, so it is smooth and easily cleanable. Observed walls of walk-in cooler unclean with black dirt. Clean and maintain all walls and ceilings of the walk-in cooler. #55 instructed to clean and maintain floors along the wall base, especially at the rear by the ice machine and utility sink. Priority foundation violation 7-42-090, citation issued.
