- comments: 6-202. 15 outer opening not protected: found both rear screen doors not completely enclosed to door frame(gaps of 1/4# to 1/2' bottom, side and top)also screen mesh broken with holes. Observed rolled-up door between kitchen and front serving area(where door is not protected) remains open at all times. Instructed to repair cause by providing window with screen mesh to pick up food from kitchen

- comments: 3-305. 11 waste pipe a few inches from the prep line cooler in the kitchen must be encased. Surface must be smooth, cleanable, and non-absorbent surface

- comments: 2-402. 11 not all employees have restraint hair in the prep area/dish room. Instructed to provide.

- comments: 3-304. 14 must store wiping cloths in prep areas in sanitizer bucket between uses to prevent cross-contamination. Instructed to maintain.

- comments: 4-501. 12 some cutting boards in the kitchen with dark stains and deep cuts, must refinish or replace said cutting boards

- comments: 4-302. 13 noted no irreversible registering temperature indicators for high-temperature dish machine. Instructed to provide an irreversible registering temperature indicator, such as strips/stickers or thermometer. Priority foundation violation: 7-38-005. No citation was issued.
