- comments: found 4 dead roaches under employee unisex toilet room handwashing sink in cabinet. Also noted dead roches throughout employee dressing room. Instructed manager to remove dead roaches', clean sanitize and maintain said area.

- comments: found shelves located in walk in cooler filled with white and grey mold like substance. Instructed manager to clean sanitize and maintain or replace.
51. Plumbing installed; proper backflow devices
- comments: found handwashing sink installed directly over utility sink. Observed handwashing sink drain pipe draining into utility sink. A plumbing system shall be designed, constructed and installed according to law. Priority violation. 7-38-030(c). Citation issued.

- comments: found no covered garbage receptacle located in 2 out of 2 employee toilet rooms. Instructed manager to provide.

- comments: found inadequate lighting located in unsisex employee toilet room. Also noted burnt out light bulb located in dry storage room. Instructed to provide adequate light intensity to said areas.

- comments: found continued non compliance. The following previous core violation was not corrected by correct by date from inspection report #2521761 on june 21, 2021: unable to locate backflow prevention device on coffee makers, instructed to keep in easy to view area. Priority foundation violation. 7-42-090. Citation issued
