16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice machine located in the basement food prep area with a build-up of black slime substance, dripping onto ice cubes stored at below ice bin. Ice is used for human consumption. Instructed to clean,sanitize and maintain. Priority foundation violation #7-38-005. Citation issued

38. Insects, rodents, & animals not present
- comments: observed a 1/4' gap at the bottom of the rear exit door leading to the alley. Must provide a tight fitting seal at all outer openings to prevent the entry of pests.

39. Contamination prevented during food preparation, storage & display
- comments: observed containers of food stored on the floor in the walk-in cooler. Instructed manager to properly store and protect food items from contamination while stored by elevating at least 6 inches from the floor on raised shelving units.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed milk crates being used as shelving throughout the walk-in cooler located in the basement. Must remove and provide shelving that is 6' off the floor and provides floor accessibility to ensure adequate facility cleaning.

57. All food employees have food handler training
- comments: observed expired food handler certificates of employees on site. Instructed to renew and maintain on file at all times.

58. Allergen training as required
- comments: observed the certified food manager without an allergen training certificate. Management instructed to have all certified food managers obtain this certificate.

- comments: observed no records of employees vaccination status on site. Instructed to provide and maintain on file.
