Report date: Aug 17, 20213. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on the premises. Management instructed to have a signed employee health policy for each employee. Priority foundation 7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean up procedure or supplies on the premises to respond to a vomit or diarrheal event. Management instructed to provide both. Priority foundation 7-38-005.
9. No bare hand contact with rte food or a pre-approved alternative procedure properly allowed
- comments: observed bare hand contact with ready-to-eat foods while food handling employees were preparing a burrito and tlayuda. Observed a food handling employee take the burrito off the grill with their bare hands and observed another employee putting ready-to-eat ingredients on a tlayuda with their bare hands. Management instructed that no bare hand contact is allowed with ready-to-eat foods. Priority 7-38-010. Citation issued.

- comments: observed no hand soap at the hand washing sink next to the 3 compartment sink in the rear food prep area. Management instructed to provide hand soap at all hand washing sinks at all times. Priority foundation 7-38-030(c). Citation issued.

- comments: observed no paper towels at the exposed hand washing sinks at the wait station between the 2 prep area and in the rear food prep area. Management instructed to provide paper towels at all hand washing sinks at all times. Priority foundation 7-38-030(c). See violation #10 above for citation.

- comments: observed no consumer advisory disclosure statement on the menu for foods that can be ordered raw or undercooked. Must add a statement before the consumer advisory stating that 'these items can be ordered raw or undercooked. ' priority foundation 7-38-005.
38. Insects, rodents, & animals not present
- comments: observed 5 live house flies flying around the dining room and prep area and landing on food contact surfaces such as cutting boards and colanders. Also observed 10 small flying insects in the washroom and rear food prep area on the walls. Additional pest control service is needed to eliminate the pest activity. Priority foundation 7-38-030(c). Citation issued.

- comments: observed rusted shelves inside of the 3 door prep cooler in the rear food prep area. Must scrape and repaint or replace the rusted shelves.

- comments: observed no chemical test strips on the premises to measure the sanitizer concentration at the 3 compartment sink. Must provide. Priority foundation 7-38-005. Citation issued.

- comments: clean the interior surfaces inside of the 3 door prep cooler.

- comments: repair the faucet at the 3 comparment sink that is leaking and spraying water.

- comments: replace the broken floor tiles under the grill in the front food prep area.

- comments: previous core violations from report #2365639 on 3/12/20 not corrected: 57 - no food handler certificates were provided for any food handling employees. Must show proof of training for all employees. 58 - observed the certified food manager without an allergen training certificate. Management instructed that all certified food managers must show proof of training. Priority foundation 7-42-090.

- comments: observed no inspection report summary posted on the premises. Management was given a copy of today's inspection report and was instructed to post it in public view. Priority foundation 7-42-010(b).
