- comments: observed no pest control log book provided from a licensed idph pest control company. Management instructed to provide a pest control log book with a recent pest control inspection from a licensed idph pest control company. Priority foundation 7-38-020(c). No citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed meat stored in a plastic grocery bag in the prep cooler. Management instructed to use only food grade storage bags or containers.

49. Non-food/food contact surfaces clean
- comments: clean the grease trap, prep tables, food storage containers, interior and exterior of the chest freezer and prep coolers, steam table, and deep fryer.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle in the employee washroom. Management instructed to provide.

55. Physical facilities installed, maintained & clean
- comments: clean the food dberis on the floor behind the 3 compartment sink.

- comments: clean the grease on the ventilation hood and filters.

- comments: observed a certified food manager without an allergen training certificate. Management instructed that all certified food managers must show proof of training.

60. Previous core violation corrected
- comments: previous core violation from report #2355271 on 12/12/19 not corrected: 53 - observed the self closing device on restroom missing. Instructed management to install a self closing device on restroom door. Priority foundation 7-42-090. No citation issued.
