- comments: observed no vomit and diarrheal clean-up procedure and kit on site. Must provide and maintain. Foundation violation 7-38-005 no citation issued
38. Insects, rodents, & animals not present
- comments: observed approximately 20 small flies around dumpsters located in garbage room containing walk-in freezer. Contact pest control and eliminate all small fly activity. Must separate trash disposal and food storage. Priority foundation violation 7-38-020 (a) citation issued

- comments: observed walk in freezer located in garbage room containing dumpsters. Food can not be stored in garbage room. Relocate dumpsters to areas not containing food.

- comments: observed floor of garbage-foos storage room room unclean with dirt and debris. Observed floor of stock room unclean outside walk-in freezer in corners and along walls. Observed no smooth, easily cleanable ceiling in garbage room above walk-in freezer. Observed porous drop ceiling in stock room containing dry goods and walk-in coolers/ freezers. The ceiling is unclean around ceiling vents and is not smooth and easily cleanable.
