- comments: procedure/plan for responding to vomiting and diarrheal events in place but has has kit/clean-up items as outlined in policy. (necessary items at a minimum: gloves, face masks, disposable mops and appropriate sanitizer/spray that kills norovirus). Priority foundation violation 7-38-005,no citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand soap provided at the hand washing sink in wash room. Management instructed to provide hand soap at all hand washing sinks at all times. Priority foundation violation 7-38-030(c),no citation issued today. (cos)

25. Consumer advisory provided for raw/undercooked food
- comments: observed no consumer advisory on the menu with disclosure and reminder. Instructed management to put consumer advisory on the menu. Priority foundation violation 7-38-005 no citation issued today

- comments: observed missing thermometers in coolers at rear storage area. Must provide and maintain visible thermometers in refrigeration units.

- comments: observed torn screen at rear exit door with openings,mode of entry. Instructed to repair screen door to prevent pest entry.

- comments: observed food handling employees prepare food without wearing a hair restraints. Management instructed that all food handling employees must wear a hair restraint.

- comments: observed missing sink stoppers for the dish washing 3 compartment sink area. Instructed to provide 3 adequate working sink stoppers and to maintain.

- comments: detail clean interior bottom compartment of deep fryer in prep area to remove grease build-up debris and maintain.

- comments: observed no back flow device at mop sink. Instructed management to provide a back flow device at mop sink. Core violation

- comments: observed cluttered conditions stored on floor next to rear refrigeration units. Instructed to remove unnecessary items due to potential pest harborage.

- comments: instructed to detail clean kitchen exhaust hood/filters to remove accumulations of dusts/grease build up and to maintain.

- comments: food handler (s) with no proof of food handler training certificate. Instructed to provide.

- comments: observed no allergen certification on site. Instructed management to become allergen certified. Core violation
