- comments: observed the low temperature dish machine in use and not able to properly sanitize. 0ppm of chlorine. Unit was tagged and can not be used until inspected by cdph inspector. Instructed pic to have the machine serviced so that it properly sanitizes. Priority foundation violation 7-38-025 citation issued
38. Insects, rodents, & animals not present
- comments: observed 30 small live flies on the walls, ceiling and conduits and 10 rat droppings by the ice machine in the front prep area. Instructed manager to eliminate small flies, remove rodent droppings, clean/sanitize the affected areas and call pest control for assistance (must be serviced before reinspection date). Priority foundation 7-38-020(a) citation issued
40. Personal cleanliness
- comments: observed employees in the food prep areas not wearing hair restraints. Instructed pic to have all employees in the food prep area to wear a hair restraint. Maintain same.
43. In-use utensils: properly stored
- comments: observed large scoops used to scoop ice from the ice machine stored on top of the ice machine. Instructed pic to remove scoops, wash, rinse, sanitize and store scoops in a clean bin/container. Maintain same.

- comments: observed the lower surface of all reach in coolers/freezers and the storage racks inside the walk in cooler/freezers and prep area with food, dirt and debris. Instructed pic to clean and maintain all.
55. Physical facilities installed, maintained & clean
- comments: observed the interior of the floor drains and floors under the fryers, coolers, freezers, sinks, in the corners and in the walk in cooler/freezer with food and debris. Instructed manager to detail clean and maintain floor drains and floors. -walls, light shields and ceiling with dirt and dust build up. Instructed pic to clean and maintain all.

- comments: observed core violations from report #2579830, dated 08/09/2023 not corrected and/or maintained. Instructed pic to correct and maintain: -#38-front and rear entrance and exit doors not completely rodent proofed at the bottom as required; instructed to rodent proof all doors. -#57-instructed that all food handlers must obtain the required food handler training. -#58-instructed that all city of chicago food service managers must obtain the required food allergen training. Priority foundation violation 7-42-090 citation issued
