- comments: observed evidence of appx. 150 or more live small flies flying around and on walls and ceilings in prep area,customers soda area,women's toilet room,dining area (trash bin),opening in lower wall behind floor drain in front service counter. Recommended to have a pest control operator to service premises on or after 7/16/21. (priority foundation 7-38-020(a)(citation issued)

- comments: must clean debris build up from fryers,reach in cooler,reach in freezer,wheel bases of shelving units,interior panel of ice machine,walk in cooler-shelving units & fan guard covers,cup holders,lower shelves at service counters.

- comments: must repair or replace hole in lower wall behind floor drain at front service counter,damaged and broken floor tiles throughout prep and dish washing areas. Must grout floor in prep & dish washing areas and where needed. Loose wall paper in dining area.
