- comments: observed no employee heatlh policy on site. Instructed to obtain for all employees. Priority foundation 7-38-010

28. Toxic substances properly identified, stored, & used
- comments: 7-206. 12 observed open rodenticide bait located through out the basement area. Instructed to remove all open rodent bait only allow pest control to properly apply rodenticides. Priority violation. Citation issued 7-38-015.

37. Food properly labeled; original container
- comments: 3-302. 12 observed inadequate labels for food items stored in containers. Instructed to provide adequate labels with the common names of foods for all food that are out of the original containers.

38. Insects, rodents, & animals not present
- comments: 6-501. 112 observed a dead rodent located behind the hot water heater in the basement. Instructed to remove all dead rodents and clean and sanitize affected areas.

- comments: 3-304. 12 observed food contact utensils improperly stored between the pipes located by the air gap for the 3 compartment sink. Instructed to store all utensils properly at all times.

- comments: 5-204. 12 observed back flow prevention device is needed on the faucet for pot filler with hose attached or remove hose attached. Instructed to provide.

- comments: 6-201. 11 observed floors need to be re-grouted through out the various areas of the food prep area. Instructed to make all surfaces smooth and easily cleanable and maintain.

Instructed to remove all unnecessary items and elevate everything at least 6 inches from the ground. Observed milk crates being used for storage. Instructed to remove milk crates and provide proper storage shelves.

- comments: observed no allergen training for city of chicago certified manager. Instructed to obtain and provide on site.

59. Previous priority foundation violation corrected
- comments: 8-404. 13(b:3) observed previous priority foundation violation not corrected from inspection report #2501006 dated 5/3/21; observed rodent activity on premises. Around 50 rodent droppings in the food prep area under cool prep stations, near 3 comp sink, along wall bases. Around 60 scattered throughout basement on the floor by the deep freezer, in closet and in far rear storage room; instructed to clean all affected areas. Reccomended contacting pest control. During re-inspection on 5/11/21 observed about more than 50 rodent droppings in the food prep area, more than 100 rodent droppings on the stair case leading to the basement and more than 100 rodent droppings located in the basement in between equipment and on the shelves and more than 40 rodent droppings located in the furnace room and more than 20 rodent droppings located in the office in the basement. Also observed live insect activity in the food prep area. Observed about 20 live ants crawling under the 3 compartment sink. Instructed to clean and sanitize affected areas, eliminate all rodent and insect activity and contact pest control for service. Priority violation. Citation issued 7-42-090.

have an incorrect ingredient label. The product was distributed nationwide and sold in specialty retail stores, grocery stores, and other retail outlets. The recalled products were distributed from 03/01/2024 to 03/29/2024. No other products from this company are affected.

Sea salt almond alligators contain almonds as an ingredient, which was not declared on the ingredient label. People who have an allergy or severe sensitivity to almonds run the risk of serious or life-threatening allergic reaction if they consume these products.

Retail products are packaged in candy boxes with a label on the bottom of the box containing nutrition and ingredient information. The code 0315 is found in a box at the bottom center of the ingredient label. The company has not been notified of any illness associated with this product.

Consumers who purchased the product/code cited above are advised not to eat any product and should destroy or return the product to the place of purchase.
