- comments: observed no consumer advisory for raw or undercooked foods. Instructed to identify such foods by asterisking them to a foot note that states: 'items can be served raw or undercooked or contain undercooked ingredients' and add a statement that says: 'consuming raw or undercooked animal products may increase your risk of food borne illness, especially if you have medical conditions'. Priority foundation 7-38-005.
38. Insects, rodents, & animals not present
- comments: found no pest control log book on site. Instructed to keep inspection receipts (at least twice a year per code) on-site at time of inspection and a pest control license with an idph 051 number. Priority foundation 7-38-020(c).
44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: found ice scoops at both ice machines stored on dirty surfaces. Must store utensils in a clean, dry location that is not exposed to sources of contamination.
55. Physical facilities installed, maintained & clean
- comments: floor of walk-in liquor storage trailer is made of unsealed wood. Must seal wood to provide a non-absorbent, smooth, easily cleanable surface.
60. Previous core violation corrected
- comments: previous core violations from 8/2/2019 report #2304128 not corrected: #51 (5-202. 11) must install a grease trap to be connected to the 3 compartment sink in the food prep areas. #55 (6-201. 11) observed raw wood used for the bar. Must seal with a high gloss finish to provide a non-absorbent, smooth, easily cleanable surface. #58 (2-102. 13) must provide allergen training certification for all certified managers and maintain certificates on site.
