- High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Employee preparing take out orders not wearing gloves dropped pans on the floor and took the pans to the dishwash area then returned to preparing take out orders and did not wash hands or put on gloves

- High Priority - Rodent activity present as evidenced by rodent droppings found. Observed 17 rodent dropping behind reach in freezer chest next to kitchen. Employee swepted and discarded **Warning**

- High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. "Y" connector at mop sink is missing vacuum breaker on the non-chemical side of the "Y" connector. Owner removed Y splitter **Corrected On-Site**
