- comments: rear exit screened door noted with open 1' gaps on the bottom threshold and sides. Also torn mesh screen of the door with an open hole. Must provide a tight fitting screened door on all sides and no open holes in mesh screening.

41. Wiping cloths: properly used & stored
- comments: noted dirty wiping cloths on prep tables at front and rear prep areas. Not properly stored. Instructed to properly store all washcloths in a sanitizing solution especially when not in use.

43. In-use utensils: properly stored
- comments: must not store in use utensils on the faucet of the hand washing sink.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: must not store clean utensils and food containers on rear patio. Must store indoors on clean surfaces.

- comments: must not use milk crates in the kitchen as a means of elevation.

- comments: must not use cardboard or paper as liner for shelving in the rear kitchen. Shelving must be smooth and cleanable. ----must not use raw wood as a splash guard on any shelving or equipment.

- comments: floors noted throughout the kitchen under and behind coolers and freezers, sinks, wok cooks line, under all shelving with food debris, grease, dead roaches and dirt. ----walls noted throughout the kitchen with food splatter and grease. Must clean all and maintain.

- comments: previous core violation from report #2472618 on 1-12-21 not corrected. #47- bottom shelving of the tall reach-in freezer in poor repair. Broken/cracked with exposed insulation. Must replace/repair to be smooth and cleanable. Priority foundation violation.
