- comments: observed previous priority violation from report #2582924 dated:10/05/2023 not corrected; #60 observed previous core violations from report #2570999 dated: 2/8/2023;# 37 found bulk containers not properly labeled with the name of the product inside of the container; must label containers and maintain. #38 observed approximately 100 mouse droppings in the following areas; behind counter area on sides and underneath shelves, in storage room next to counter area on floor, in dry storage area in rear kitchen, under all shelving in kitchen, next to hot water heater in rear kitchen, behind cooking equipment in kitchen area. Instructed person in charge to remove all mouse droppings, sanitize all areas, and inform pest control of activity. Observed 4 live roaches in kitchen area on top of ice machine. Instructed person in charge to eliminate roach activity. #49 instructed to clean and maintain the following equipment: bottom of fryer, exterior of fryer, wok station, steamer to remove grease. Also clean interior panel of ice machine. Also clean exterior surfaces of bulk containers. #55 instructed to clean walls and floors around, under and behind equipment to remove dirt, grease and rodent droppings. Instructed to clean the ceiling vents in the kitchen to remove dust, clean vent on the ceiling in the washroom to remove dust. #58 all city of chicago food service managers must obtain the required food allergen training. Instructed person in charge to come into compliance with all violations and maintain. Instructed to come into compliance. Priority violation 7-42-090. Citation issued.
