Went to Chipotle on Victory Blvd next to Blaze Pizza around 4:30pm on 2/2/18. Ate the food. Woke up around 3:30am with diarrhea, stomach cramps, nausea and vomiting.

Had steak burrito w/ white rice, hot salsa, corn salsa, cheese, sour cream, and guac on Friday at 3pm. Became very ill that night with a fever and severe diarrhea.
