1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed no person-in-charge present with city of chicago certified manager certificate.

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on site or valid documentation while (tcs foods) (deli meat sandwiches etc. )are being prepared/served to the customers. Instructed person in charge, a certified food manager with city of chicago foodservice sanitation certificate must be present while preparing handling and or serving (tcs foods). Priority foundation violation 7-38-012, citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the handwashing sink in the food prep/dishwashing area not accessible for use. Hand sink being blocked with multi-use comntainers and found storing bags of frozen meats inside said hand sink while food handlers were actively preparing foods at this time. Instructed manager, handwashing sink must be accessible for employees to wash hands at all times. Person-in-charge removed. Priority foundation violation 7-38-030 (c), citation issued.

- comments: observed no hand soap provided at the handwashing sinks at the prep/dishwashing area and inside the washroom. Instructed to provide and maintain hand soap at all handwashing sinks at all times. Priority foundation violation 7-38-030(c), violation consolidated with the above citation.

- comments: observed no hand paper towels provided at handwashing sinks at the prep/dishwashing area and inside the washroom. Instructed to provide and maintain hand paper towels at all handwashing sinks at all times. Priority foundation violation 7-38-030(c), violation consolidated with the above citation. Cos

- comments: observed improper thawing being done with frozen beef/chicken. Thawing must occur with food being placed in the cooler over night, or under cold running water. Instructed to maintain correct thawing procedures and maintain.

- comments: instructed to label repackaged food items on display for sale and maintain.

38. Insects, rodents, & animals not present
- comments: observed sticky fly trap roll hanging at ertrance of the prep/dishwashing area. Instructed to remove and maintain.

49. Non-food/food contact surfaces clean
- comments: instructed to detail clean the grease buildup underneath the flat-top cooking grill and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle provided in the washroom. Must provide and maintain.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside garbage dumpster with overflowing trash lids preventing proper lid closure. Instructed to increase service pick-up or larger garbage dumpster must be able to close and remain tigh fitting in order to prevent rodent harborage and maintain. Priority foundation violation 7-38-020(b), citation issued.

55. Physical facilities installed, maintained & clean
- comments: observed food debris and spills on the floors and surfaces throughout the food prep/dishwashing area instructed to detail clean and maintain at all times. Observed missing and damaged floor tiles throughout the food prep/dishwashing area and maintain so to smooth, easily cleanable surfaces also, remove cigarette butts on the floor located in the basement storage area.

55. Physical facilities installed, maintained & clean
- comments: all unused equipments/articles stored in the basement storage area must be removed from the premises and all other need items must be elevated at least 6" inches off the floor to facilitate cleaning and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: observed no ventilation over the table-top cooking grill. Instructed to provide adequate ventilation and maintain as required.

57. All food employees have food handler training
- comments: observed food handler(s) with no proof of food handler training certificates. Instructed to provide and maintain as required.

60. Previous core violation corrected
- comments: previous core violations not corrected from inspection report#2355654 dated:12/19/2019 #51---5-203. 13: **c** service sink premises has no mop sink. Instructed manager to install and maintain with hot and cold running water. Also provide and maintain a back flow device. #52---5-402. 12: **c** grease trap instructed manager to provide and maintain a grease trap for the 3 compartment sink. #53---6-501. 19: **c** closing toilet room doors instructed manager to provide and maintain a self closing device on the wash room door. Instructed to correct noted vilations and maintain by 4/9/2026. Priority foundation violation 7-42-090, citation issued.
