- comments: observed gap opening approx 1/4 inch at bottom left corner of the rear exit door. Must pest proof door all across the bottom to prevent pest entry and maintain.

- comments: observed chipped rust on the wire storage racks in the walk-in cooler. Instructed to remove rust and repaint or replace the shelves.

- comments: observed outside waste grease container not maintained. Grease container had grease all on top of the lid and spillage on to ground floor. Instructed to detail clean due to potential pest feed and maintain. Priority foundation violation 7-38-020(b),citation issued.

- comments: instructed to detail clean and organize the rear hot water tank area and remove unnecessary articles due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6' inches to facilitate cleaning.
