- comments: observed no certified food manager on
duty while tcs foods are being prepared,handled and served such as (pork,ham,chicken,rice,etc)must be on site at all times. (priority foundation 7-38-012)(citation issued)

- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)

- comments: observed evidence of appx. 40 or more rat droppings on floor under carts in basement. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 3/29/21. (priority foundation 7-38-020(a)(citation issued)

- comments: must clean debris build up on filters and hood at cooking equipment.

- comments: previous core violations from report # 2352655 12/3/19 not corrected. #36-instructed to properly label all food items with product name including bulk food containers. #47- observed both cutting boards in the food prep are to have open seams and dark stains. Instructed to replace and/or refinish damaged or stained cutting boards. #55- observed clutter in the basement. Instructed to clean and organize clutter in the basement. #57- observed no food handler certificate for employee on site. Instructed to provide food handler certificates for each employee on site.
