- comments: observed drink/ ice dispenser located next to register with a build up of black, brown and pink substance on the top interior of ice reservior and a thick pink, black and brown slime substance on surfaces surrounding ice chute. Ice is used for human consumption. Instructed to remove ice, clean and sanitize all surfaces and maintain. Priority foundation violation 7-38-005. Citation issued

- comments: observed ready-to-eat, time/temperature control for safety (tcs) foods (bacon, sausage patties, sausage links, rice, garden salad, turkey bacon) not date marked to indicate the product's name and date in which the food must be sold, discarded or consumed within 7 days. Instructed to provide proper labels for date marking and discard date for refrigerated, ready-to-eat, tcs foods held over 24hrs placed in all cold-holding units. Person in charge discarded approx. 50 lbs valued at $75. Priority foundation violation #7-38-005. Citation issued

- comments: noted handsink next to fountain drink and slushy machine with leak coming from the drainpipe. Instructed to repair

- comments: noted clutter in office located in prep area/dishroom. Instructed to declutter. Removing items unnecessary to business. All kept items are to be stored on proper shelving that is a minimum of 6 inches off the ground to prevent pest harborage and ensure adequate cleaning of facility
