- comments: no city of chicago certified food manager on site while tcs foods such as beef sauce were being prepared and served. Priority foundation violation 7-38-012 citation issued.

- comments: menu does not disclose and inform consumers of the specific menu items that are raw or undercooked and a potential hazard of consuming such food. Must provide a consumer advisory on the posted menu that discloses and reminds consumers of foods such as eggs. Priority foundation violation.

- comments: inoperable internal thermometer inside the front two-door milk cooler. Must replace.

Must clean and maintain. ----wall caulking at the three-compartment sink in poor repair, peeling, moldy. Must replace. Must not use expanding spray foam as a sealant/caulking at the three-compartment sink. Must be smooth and cleanable.

- comments: must provide shatterproof bulbs or light shields for lights above the three-compartment sink.
