- comments: the person in charge at the time of inspection does not have a city of chicago sanitation certificate. Priority foundation violation. No citation issued -
2. City of chicago food service sanitation certificate
- comments: no original and valid city of chicago food service sanitation certificate was posted nor provided on-site for the present manager on duty. Instructed to provide. A least one certified food manager must be on site when food is handled(hot dogs, taquito, chicken etc). Priority foundation: 7-38-012, citation issued.
38. Insects, rodents, & animals not present
- comments: observed two live rats at rear dishroom area going under the door equipment room by exit door. Instructed to contact pest control. Must clean and sanitize throughout the premises. Priority violation:7-38-030(a), citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: splash guard installed between handwashing sink and prep front counter no long enough to protect prep counter, instructed to correct issue. The surface must be smooth, cleanable, and non-absorbent surface.

- comments: must not use milk crates as a means of elevation for stored items and to use for seating. Must remove and use raised shelves and a chair.

- comments: no chemical test strips on-site to check sanitizing levels for the three-compartment sink at the rear dishroom. Must provide chemical test strips according to sanitizer solution. Priority foundation violation 7-38-005 citation issued.
