- comments: observed a 1/2' gap along the bottom and middle of the 2 entrance doors. Management instructed to seal all outer openings to protect against the entry of pests.

38. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity and live flying insects on the premises. Observed approx. 50 mouse droppings on the floor and storage shelves in the small corner storage room next to the walk-in freezer, along the walls under the storage shelves in the rear storage/dish washing area and behind the atm machine and ice cream freezer next to the front door. Also observed over 10 live small flying insects flying around the dish washing area and on boxes of stock. Managemement instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate all pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed many dead insects on the floor and storage shelves of the dish washing/storage area. Management instructed to clean and remove all dead insects from all affected areas.

- comments: replace the torn gasket inside of the 1 door prep cooler.
