2. City of chicago food service sanitation certificate
- comments: observed facility food handler preparing and handling, time and temperature control for safety(tcs) foods (hotdogs, pizza, and milk) without original city of chicago food service manager and certificate on site. Informed person in charge city of chicago original food service certificate must remain posted on-site and certified manager on-site during tcs food prep, cooking, serving, and operating hours. Priority foundation violation #7-38-012. Citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on-site for all food employees. Priority foundation violation #7-38-010. No citation was issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the following sinks not maintained with hot water: improper temperature of hot water observed at front hand sink near the coffee station (76f). Instructed person in charge of hot water at all handwashing sinks must be at least 100f. Priority violation #7-38-030(c). Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed hand sink in rear dish area not accessible. Basin was filled with utensils, boxes, and containers in the front of the sink. Instructed that sink must be accessible and maintained at all times. Priority foundation #7-38-030(c). Citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed facility equipment for cold holding in disrepair. Noted reach-in prep cooler to be at improper at 47. 5f. Informed person in charge to repair or replace for proper cold holding of 41 degrees f or below. Tagged unit. Priority violation #7-38-005. Citation issued.

49. Non-food/food contact surfaces clean
- comments: observed food debris and splash all throughout the floor of the walk-in cooler. Instructed to detail clean and maintain.

52. Sewage & waste water properly disposed
- comments: observed inadequate waste and sewage water disposal from the floor drains in the rear dish area/ dry storage room and employee toilet room. Must remove water from floor, request a licensed plumber to repair, keep dry, and maintain at all times. Priority violation #7-38-030(c). Citation issued.

55. Physical facilities installed, maintained & clean
- comments: observed clutter of boxes, equipment, and single-service items throughout the rear dry storage/dish area. Instructed to organize and clean to prevent harborage of pests.

57. All food employees have food handler training
- comments: observed no paper copy food handler certificates for all employees. Instructed to maintain records at all times.
