- comments: observed person in charge without chicago food sanitation certificate on site. Instructed to obtain certificate and maintain on site.

2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food service manager nor city of chicago sanitation certificate on site while tcs foods are being prepared and served. Such as chicken wings, hot dog, chicken taquitos etc,. Instructed above that certified food service manager has to be present with original city of chicago certificate at all times tcs foods are being prepared and served. Priority foundation violation 7-38-012. Citation issued.

- comments: no covered waste receptacle provided in the women's washroom. Must provide and maintain.
