- comments: the person in charge at the time of inspection does not have a city of chicago sanitation certificate. Priority foundation violation. No citation was issued.
2. City of chicago food service sanitation certificate
- comments: no employee on site with a valid city-certified food manager certificate or valid documentation provided on site of any individual pending to receive a city manager certificate on-site while food is being served to the public on-site. Priority foundation violation 7-38-012 citation issued.
36. Thermometers provided & accurate
- comments: thermometers is not provided in walk-in cooler. Cold/hot holding equipment used for time/temperature control for safety shall have at least one thermometer in an easily viewed location.
38. Insects, rodents, & animals not present
- comments: observed approximately over 30 small black flying insects scattered throughout the kitchen facility, near coffee station, near soda dispenser, and near 3 copartment sink. The facility must ensure all small black flying insects are removed from the premises, all areas are cleaned and sanitized, and a licenced pest control company treats the facility for insect activity. Priority foundation violation 7-38-020 (a) citation issued.

- comments: backflow prevention device not found on mop sink. Facility must provide.

- comments: unnecessary items / nonfunctional equipment behind soda box lines in the back storage area. The premises shall be free of items unnecessary to the operation or maintenance of the establishment. This includes equipment that is nonfunctional or no longer used.
