2. City of chicago food service sanitation certificate
- comments: observed (pic) manager on duty with no valid city of chicago food sanitation certificate posted at this time. Tcs foods being prepared, sandwiches being made,etc. Instructed that certified food manager must be on site at all times tcs foods are prepared and handled. Priority violation issued 7-38-012.
38. Insects, rodents, & animals not present
- comments: observed evidence of improper pest control on site. Observed 29-mice droppings left inside bottom red cabinet in washroom,21 mice droppings on floor under front counter,4- droppings on top of brick on floor near walk in cooler. Instructed to throughly clean and sanitize all areas. Recommened pest control service. Priority foundation issued 7-38-020(a).
49. Non-food/food contact surfaces clean
- comments: observed hood filters of cooking equipment not clean. Instructed to clean/maintain.
51. Plumbing installed; proper backflow devices
- comments: observed leaky faucet at 3- compartment sink. Instructed to repair
