- comments: observed interior of the ice machine unclean with a black mildew-like substance. Ice is used for human consumption. Must clean in detail the interior of the ice machine where ice is made and stored and maintain the ice machine. Must discard all contaminated ice. Priority foundation violation 7-38-005

- comments: observed over 100 rodent droppings throughout the facility, including behind stove coolers, in the hot water heater room, and the rear storage room behind the kitchen. Must remove all rodent droppings, clean and sanitize all affected areas and maintain the facility. Must obtain recent pest control service. Priority foundation violation 7-38-020(a)

- comments: observed rear storage room containing too much clutter and harborage. Must remove all non-restaurant-related items and all non-working items from facility. Must maintain all remaining items at least 6 inches away from walls and up off the floor.
