- comments: 2-101. 11. Pic doesn't have a certified food manager's certificate. Must provide and maintain. (priority foundation 7-38-012)

- comments: 2-101. 11 observed no certified food manager on duty while tcs foods are being prepared, handled, and served, such as (pork, cabbage, slicing deli meats, etc. )must be on site at all times. (priority foundation 7-38-012)(citation issued)

- comments: 3-302. 12 : must label food storage conatiners when food is not in original package (shakers,containers,bulk,squeeze bottles,sprayers,etc).

- comments: 6-501. 111 :previous priority foundation violation from report #2622679 8/22/25 not corrected #38- still observed evidence of appx. 200 or more live small flies flying around prep and dish washing areas,walls and ceilings ,produce display area on onions,rear storage area in bags of onions,mopsink areastaff toilet room,sales floor. Also appx. 25 or more live large flies on baking and food prep tables,flying around premises. Recommended to have a pest control operator to service premises on or after 9/3/25. (priority 7-42-090)(citation issued)
