- comments: observed no certified manager on duty while tcs items such as pierogis were prepared. Instructed to maintain an employee holding a city of chicago foodservice manager certificate on site at all times.
Priority foundation violation 7-38-012. Citation issued.
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no verifiable employee health policy on site at the time of inspection. Left template and instructed to maintain copies of health policy signed by all employees on site at all times. Priority foundation violation 7-38-010. Citation issued.

- comments: observed no kit on site for responding to vomit and diarhea. Left one pager and instructed to maintain kit containing all necessary supplies, including a sanitizer rated as effective against norovirus on site at all times. Priority foundation violation 7-38-005. Citation issued.

- comments: observed no handwashing signage in employee toilet room located in hallway behind coffee station and in basement. Instructed to provide.

- comments: observed chicken in hot hold case at deli counter at improper temperature 123. 8f. Management discarded approximately 5 lbs of product valued at approximately $30. Instructed to maintain all hot tcs items above 135f at all times. Priority violation 7-38-005. Citation issued.
22. Proper cold holding temperatures
- comments: observed the various tcs items held in prep coolers in the rear kitchen area at the stated improper temperatures: deli meat at 59. 8f, sausage at 62. 3f, and sausage at 63. 5f. Management discarded approximately 200 lbs of products valued at approximately $600. Instructed to maintain all tcs items at or below 41f at all times. Priority violation 7-38-005. Citation combined with the above violation.
33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed 2 prep coolers containing tcs items, such as deli meat and sausage, maintaining improper ambient temperatures of 62. 6f and 54. 5f. Both coolers were tagged and held for inspection. Instructed to repair or replace and maintain all coolers at or below 41f at all times. Priority violation 7-38-005. Citation issued.

- comments: observed 20 dead cockroaches in the basement. Instructed to remove and maintain facility free of dead pests.

- comments: observed torn gasket on cooler located opposite walk-in coolers. Instructed to repair or replace and maintain in good condition.
