- comments: pic doesn't have a certified food managers certificate must provide and maintain. (priority foundation 7-38-012)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods are being prepared,handled and served such as (chicken,mashed potato,slicing deli meats,etc)must be on site at all times. Manager arrived on duty at 10:55 a. M. (priority foundation 7-38-012)(citation issued)(cos)

9. No bare hand contact with rte food or a pre-approved alternative procedure properly allowed
- comments: observed bare hand contact with ready to eat tcs foods such as pancakes. Observed female food handler remove pancakes from grill in rear bread baking prep area with bare hands. Instructed to provide gloves or utensils to handle tcs foods. (priority 7-38-010)(cos)(citation issued)

- comments: observed no hand washing sink in rear bread baking/pancake prep area/dish washing area. Were i observed bare hand contact with ready to eat food and dish washing sink must install a hand washing sink with hot & cold running water,soap & towels and maintain. - comments: observed improper temperatures of tcs foods such 8lbs. Cooked mushroom soup-102. 3f. 11lbs. Cooked pierogi-112. 7f. 6lbs. Cooked potato-125. 5f. 7lbs. - comments: 6-202. 13 observed an insect zapper stored on prep counter in deli area must be removed and maintained properly.

Also appx. 20 or more live large flies on baking and food prep tables,flying around premises. 2- rat droppings on floor next to chest freezer in rear storage area. Must remove rat droppings clean and sanitize area recommended to have a pest control operator to service premises on or after 8/22/25. (priority foundation 7-38-020(a)(citation issued)

- comments: 3-305. 11 : observed food stored on floor in walk in coolers and freezers must elevate and maintain.

- comments: 4-501. 11 : must repair or replace worn door gaskets on main kitchen 1- door reach in cooler & bakery 2- door reach in cooler.

- comments: observed outdated & discolored quat chemical test strips to check sanitizing solution ppm's at 3- compartment sink when tested the strip color ppm's is blue and should show hues of green. Must provide and maintain. (priority foundation 7-38-005)(citation issued)

- comments: 4-601. 11(c) : must clean food debris and grease from prep tables, fryer cabinets,floor mixers,reach in coolers and freezers.

- comments: 5-204. 12 : observed no backflow device on ice machine in basement to be seen for servicing must provide and maintain.

- comments: must reattach self closing device on staffs 1st fl. Toilet room door leading into prep area to close tightly.

- comments: 6-501. 13 : must clean dust and food debris build up from floor under display shelves on sales floor ,prep areas and storage areas.
