- comments: person in charge at the time of inspection does not have a city of chicago sanitation certificate. Priority foundation violation. No citation issued

- comments: no original and valid city of chicago food service sanitation certificate posted nor provided on site for present manager on duty. Instructed to provide. A least one certified food manager must be on site when food is handled(eggs,milk, priority foundation violation: 7-38-012, citation issued.

- comments: employee health policy/training on site,however not signed by employees. Instructed facility to establish an appropriate employee health policy/training system and maintain with verifiable documents on site,included employee signature. - comments: 2-501. 11 observed vomiting and diarrhea incident clean up policy in place however no kit on site. Must provide. Priority foundation violation. 7-38-005. No citation issued

- comments: broken rubber gasket inside the two door reach-in freezer,also at interior of walk-in cooler door. Broken cabinet door under washbowl inside women's washroom. Reair/replace

- comments: observed that low temp dish machine concentration of chlorine (0ppm). Unit presently in use(resident multi-use utensils from breakfast). The sanitizer bucket under the unit was empty. Upon my request employee installed a new bucket with sanitized solution. Presently unit maintain 100ppm. Cups, dishes etc. Dishes rewashed and sanitized by the dishwasher. Priority foundation violation:7-38-005, citation issued

- comments: black slime substance inside the frame door of walk-in cooler, clean and maintain

- comments: noted wastewater with soap suds backing into 2 compartment food prep sink in kitchen, when handwashing sink is in use for handwashing. Instructed to repair and maintain. Priority violation 7-38-030(c). Citation issued.

- comments: observed several black plastic bags filled and tied on pavement also excess unused items outside area by the shed/staircase. Instructed to remove to prevent rodents and insects activity.

- comments: grimes and debris under the ice machine(accumulation of debris and items), coffee station, inside the walk-in cooler under shelves, in the kitchen. Clean and maintain.
