3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on the premises. Management instructed to provide a signed employee health policy for each employee. Priority foundation 7-38-010.

16. Food-contact surfaces: cleaned & sanitized
- comments: no chlorine sanitizer was detected at the final rinse for the low temperature dish machine in the kitchen. Management instructed to provide a final rinse of 100 ppm chlorine at the dish machine. Priority 7-38-025.

25. Consumer advisory provided for raw/undercooked food
- comments: no consumer advisory disclosure or reminder statement was provided on the menu for raw or undercooked foods. Management instructed to disclose or describe which foods can be ordered raw or undercooked and then link those items to a reminder statement on the menu. Reminder statement should include that 'these items can be ordered raw or undercooked. ' priority foundation 7-38-005.

38. Insects, rodents, & animals not present
- comments: observed approximately 150 mouse droppings on the floor beneath the stage area, on the steps and floor in the rear storage closet, and on the staircase to the basement beneath the wire storage rack. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation 7-38-020(a). Citation issued.

38. Insects, rodents, & animals not present
- comments: observed 3 dead mice on the glue board beneath the stage area. Management instructed to remove the dead rodents.

43. In-use utensils: properly stored
- comments: observed the ice scoop stored on top of the ice machine. Management instructed to store the ice scoop in a clean container.

49. Non-food/food contact surfaces clean
- comments: clean the interior of the microwave and the grease on the sides of the grease trap in the basement.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed excessive garbage and litter on the ground surrounding the garbage dumpster. Management instructed to clean and maintain the outside garbage area at all times. Priority foundation 7-38-020(b). Citation issued.

55. Physical facilities installed, maintained & clean
- comments: clean the grease on the basement floor around the grease trap. Clean the dust on the ceiling tiles in the kitchen prep area around the supply vents. Clean the wall behind the grill.

56. Adequate ventilation & lighting; designated areas used
- comments: must provide a light shield around the light bulb above the stoves.

60. Previous core violation corrected
- comments: previous core violation from report #2300677 on 7/3/19 not corrected: 58 - noted certified food managers on duty with no allergen training certification. Instructed management that all food service managers are required to have the allergen training certification. Priority foundation 7-42-090.
