- comments: observed no hand washing sinks in north and south serving areas. Instructed manager to install hand washing sinks with hot and cold running water under city pressure, soap, paper towels and signage in both serving areas. Priority foundation 7-38-030(c)

- comments: observed over 20 mice droppings scattered on floor in corner near blue chill reach-in cooler in prep area, inside compartments under steam table in south serving area, on floor and equipment in unused dish washing area near north serving area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed water and excessive debris on floors under equipment, along walls and in all corners in unused dish washing area near north serving area. Observed dust buildup on ceiling vents in kitchen and serving areas. Instructed manager to clean and maintain.
