- comments: observed no employee health policy on site. 7-38-010 priority foundation violation. Citation issued.

- comments: observed two door prep cooler at temp reading between 47. 6-48 degrees (holding condiments, sliced tomatoes) instructed to discard all, repair and maintain in good working order. Priority violation 7-38-005 citation issued.

- comments: observed two containers with sausage and deli meat not labeled and dated properly with common name and date. Instructed to label all. Priority foundation violation 7-38-005 citation issued.

- comments: two door cold hold unit not reading proper temperature. Ambient temperature reading 48 degrees. Unit tagged and instructed to repair and maintain a temperature of 41 degrees or less. Priority violation 7-38-005 citation issued.

- comments: observed decomposing rat located at the wall in the front food service area. Instructed to remove.

- comments: observed the front door to the facility with openings around the glass door and an opening at where it looks like a door handle was. Instructed to seal the openings as a means of preventing pest and insect entering and harboring.

- comments: observed evidence of pest activity throughout facility approx 100 plus droppings in the front food service area behind the reach in cooler, under the shelving unit at the front counter, behind the cooking equipment and inside the deep fryer area. Observed approx 100-150 droppings in the rear dish/prep area behind the large floor freezer, under the old cooling unit, inside the boiler room and in the rear dry storage area also on the food contact surfaces in the rear (cutting board, meat slicer) instructed to contact pest control and have a full service completed with proof of service log. Priority violation 7-38-020(a) priority violation, citation issued.

- comments: observed foil and card board lining cooking unit shelving in the front food service area. Instructed to remove, clean and maintain as a means of making easy to clean and sanitize area.

- comments: observed the insides of the deep fryers oil filtering/draining area with a heavy acculmulation of grease and food debris. Instructed to clean all and maintain as a means of preventing pest and insect harboring.

- comments: observed the large deep freezer unit in the rear next to slicing machine with a badly damaged top with tape on it. Instructed to repair or replace.

- comments: observed large cutting board in the rear under the meat slicer badly stained with deep cutting grooves. Instructed to replace as a means of preventing possible cross contamination of foods being prepared.

- comments: observed no sanitizing testing strips on site for testing sanitizing water at the rear dishwashing area. Instructed to provide on site as a means of properly testing sanitizing water for dishes. Priority foundation violation 7-38-005 citation issued.

- comments: observed a build up of dirt, dust, grease and food debris on the floors and walls in the front food service area under and around all cooking and non cooking equipment, and in the rear under and around the cold hold units, under the three compartment sink and storage racks in rear. Instructed to clean and maintain all areas

- comments: observed the hood ventilation system filters with a build up of grease and dirt dripping from the unit. Instructed to clean and maintain as a means of preventing possible contamination of foods being prepared.
