- comments: observed no employee health policy on site as required. Priority foundation violatioan 7-38-010. Instructed the person in charge to provide documentation of employees responsibility to report any illness transmissible through food. Citation issued.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no adequate supplies on site to properly respond to vomit and/or diarrheal events. Priority foundation violation 7-38-005. Instructed to maintain a kit with all necessary supplies including a sanitizer rated as effective against norovirus on site at all times. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed approximately 2o mice droppings in the kitchen preparation, on the bottom of prep tables, and on floor next to two door refrigerator. Violation 7-38-020(a). Instructed to remove all rodent droppings, clean surfaces, and maintain premises free of pest. Recommended receive additional pest control service. Citation issued.

- comments: observed some grease build and food debris on surface of all cooking equipment. Instructed to detail clean and maintain.

- comments: observed broken and missing floor tiles in the rear kitchen preparation area near the chest freezer and near the grill. Instructed to repair and maintain.
