- comments: observed no employee health policy on site. Instructed to provide an employee health policy that includes a signed acknowledgement from each employee. Priority foundation 7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no vomit/diarrhea cleanup kit or written procedures. Instructed management to provide. Priority foundation 7-38-005.
10. Adequate handwashing sinks properly supplied and accessible
- comments: the rear prep handwashing sink is not maintained. Sink is inoperable; water was turned off below at pipes upon arrival. Hot water handle missing; when water is turned on; water sprays through the missing handle hole. Must repair. Priority foundation violation 7-38-030(c). Citation issued.
23. Proper date marking and disposition
- comments: observed tcs foods (cooked noodles, cooked chicken, cooked egg, cooked rice, and cooked pork) held for over 24 hours with out discard date. Instructed facility to provide discard date for any tcs foods held for over 24 hours. Management voluntarily discarded said foods. Priority foundation 7-38-005. Citation issued.
58. Allergen training as required
- comments: observed no proof of allergen training on site. Must provide proof of allergen training for all chicago certified food managers.
60. Previous core violation corrected
- comments: previous core violations from 1/27/2021 report 2473197 not corrected: violation #10 (6-301. 14) observed no hand washing signs or posters near hand washing sinks throughout premises. Instructed to provide and maintain. Violation #49 (4-601. 11(b)) must detail clean excessive grease build up behind rice cooker, inside fryer cabinets. Violation #53 (5-501. 17) must provide covered receptacles in all unisex employees and customers toilet rooms. Instructed to provide and maintain. Violation #55 (6-201. 11) must detail clean dirt and grease build up under, around and behind cooking equipment. Instructed to maintain. Violation #56 (4-202. 18) must detail clean excessive grease accumulation from hood above cooking equipment in prep area. Instructed to maintain. Must correct previous core violations. Priority foundation 7-42-090.
