- comments: observed rodent activity on premises. Found 100 mouse droppings on the floor in the office area under the desk, on the floor of the walk-in cooler, on the floor on both sides of the hot water tank and on the storage shelves in the rear area. Must clean all affected areas and have pest control service prior to reinspection next week. Priority foundation violation 7-38-020(a) issued.

- comments: instructed to clean the exterior surfaces of fryers and other equipment and storage shelves.

55. Physical facilities installed, maintained & clean
- comments: observed some missing floor tile under cooking equipment thatr is filling up with grease and water; instructed to replace floor tile where needed and grout floor between tile where needed. Observed the wall base of the wall outside of the walk-in cooler and shelf area across from the three comp sink in poor repair; repair wall base. Observed the floor drain underneath the cooking equipment without a cover; instructed to provide a cover for drain.

- comments: instructed to clean floors under around and behind all kitchen equipment and in areas where rodent activity observed.
