- comments: observed no hand washig signs at hand washing sink in prep, washrooom and dish washing areas. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: observed over 60 mice droppings scattered on floor, shelves and electrical equipment in sales, rear prep and storage areas, live roach crawling on top of display cooler behind front counter, several flying insects in prep, washroom and storage areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed leaking drainpipe under hand washing sink behind front counter and leaking faucet on 3-compartment sink in rear prep/dish washing area. Instructed manager to repair and maintain.
