- comments: observed no clean-up procedure and supplies on the premises to respond to vomit and diarrheal events. Management instructed to provide both and maintain as required in policy. Priority foundation violation 7-38-005,no citation issued.

- comments: observed no thermometer inside the refrigerator. Instructed to provide and maintain a visible thermometer in refrigerator at all times.

- comments: found approximately 25 mouse droppings along walls in the rear cluttered storage areas and in furnace room. Instructed to detail clean and sanitize all affected areas and pest control service is now recommended. Priority foundation violation 7-38-020(a),no citation issued.
