- comments: instructed to properly label food containers/bins identified with common name (salt, sugar etc).
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: found milk crates being used as shelving throughout the basement area. Must remove crates from premises and provide shelving that is free of unnecessary crevices and designed/constructed to be easily cleanable.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: found the three compartment sink not maintained; sink is slow draining. When water from one compartment is drained; water backs up into adjacent compartments. Instructed management to get drainage system serviced. Priority foundation 7-38-025. Citation issued.
49. Non-food/food contact surfaces clean
- comments: found a mold-like substance covering the interior of the bread proofer, as well as on the dish trays that hold dishes in place inside the dish machine. Instructed management to detail clean the bread proofer and dish trays.

- comments: found heavy grease accumulation on the walls and ceiling tiles throughout the prep area. Instructed management to detail clean and maintain.
58. Allergen training as required
- comments: found no proof of allergen training on site for all city of chicago food managers. Instructed management that all city of chicago food managers must complete allergen training and keep certificate on site.
60. Previous core violation corrected
- comments: previous core violations from 12/9/2021 report #2545305 not corrected: violation #51 (5-205. 15) observed leaking faucet base at the 3-compartment sink. Must repair and maintain. Violation #55 (6-501. 114) observed clutter and unnecessary items in the basement. Violation #57 (2-102. 13) observed food handling employees without a food handler certificate. Must show proof of training for all food handlers. Must remove unnecessary items clean and organize area to prevent rodent harborage. Must correct previous core violations. Priority foundation 7-42-090. Citation issued.
