Report date: Apr 27, 20205. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)
38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 35 or more mice droppings on pallet along wall near mop sink,on floor in between racks,on floor behind ladder,near rear exit door. Under table in break room. Also a dead mouse on glue board near rear door. Massive amount of articles on floor along wall base throughout rear storage area could be a possible harborage area. Must remove mice droppings and dead mouse. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 4/27/20. (priority foundation 7-38-020(a)
49. Non-food/food contact surfaces clean
- comments: must clean debris build up in reach in coolers,food display shelves. Must defrost ice cream freezer.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must provide a garbage can with a lid in women's toilet room.

- comments: must clean debris build up on floor under all display shelves on sales floor and in rear storage area.
