- comments: observed no hand washing sink in serving area. Instructed manager to install hand washing sink with hot and cold running water under city pressure, soap and paper towels. Priority foundation 7-38-030(c)

- comments: observed over 20 mice droppings scattered on floor in supply room near serving area, 10 mice droppings scattered on floor along walls and corners in dry food storage area, 5 mice droppings scattered on floor in teacher lounge near refrigerator and vending machines. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed no covered waste receptacles in lunchroom staff and second floor teacher staff washroom. Instructed manager to provide.
